Under the general supervision of the VP of Operations or designee, the Clinical Communications Nurse (CCN) is responsible for triaging and facilitating safe, efficient, and effective care communications among the Med Partners team and their clients.
The position will mostly have evening work hours.
The position's responsibilities will involve working with communications platforms (i.
e.
phone, dictation, EHR, care management, etc.
) and from existing project tools and processes to developing new tools based on knowledge and experience.
Responsibilities:
Communications & Care Coordination:
Manages the company's On Call system for assigned medical providers Works in collaboration with providers to identify and address communication risks Communicates the patient care needs and information per protocol Assists team to ensure timely receipt of medical history from hospital records, PCP and/or nursing facility Cultivates and support team with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals as needed Teamwork & Collaboration:
Provides outstanding leadership to strengthen the organization's ability to deliver superior quality medical services Actively participates in the company's strategic initiatives Provides feedback for the implementation and improvement of the models of care Works collaboratively with hospital discharge teams, SHC Medical Partners and their clients' staff to ensure coordinated activities Meets physical and sensory requirements stated below, and able to work in the described environment Interprets and implements quality assurance standards Develops, maintains, and analyzes various tracking documents to facilitate the communication of clinical care and operations Compiles and distributes reports, as needed Attends required conference calls, webinars and education/training events Reports and escalates matters to executive management as needed Travels to service locations as assigned Other special projects and duties, as assigned
Qualifications:
Active Licensed Practical Nurse Minimum of two (2) years' experience as Licensed Practical Nurse, long-term care preferred Strong clinical skills including an understanding of and ability to implement evidence based care Knowledge of the health care system and its components including sites of care, delivery models, and the roles of various providers and health care professionals Experience in EHR along with workflow and practice management Experience with healthcare operations and clinical workflows Effective verbal and written English communication skills Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation Highest level of professionalism with the ability to maintain confidentiality Ability to communicate at all levels of organization and work well within a team environment in support of company objectives Customer service oriented with the ability to work well under pressure Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity Strong analytical and problem solving skills Ability to work with minimal supervision, take initiative and make independent decisions Ability to deal with new tasks without the benefit of written procedures Approachable, flexible and adaptable to change Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and clients Will work from Company's home office or remotely from home About our Line of Business:

We are seeing an experienced Licensed Practical Nurse (LPN) to become part of our team! We have several shifts available including:
Evenings 3p-11p or Nights 11p-7a (eastern), Monday-Friday OR all shifts (including days) on Weekends (Saturday or Sunday) These positions are remote/work from home, but preference will be given to Louisville area residents as there may be some travel to service locations and Corporate office training and meetings in Louisville.
The rate of pay is in the $18/hour.
